1. Consent to Participate
I, the undersigned parent or legal guardian of the above-named child, hereby give my permission for my child to participate in the One-Day Soccer Skills Camp hosted by Grace Missionary Baptist Church Inc. of Gary.
2. Acknowledgment of Risk
I understand that participation in soccer and related activities involves inherent risks, including but not limited to physical contact, collisions, trips, falls, and other accidents which may result in injury. I voluntarily assume all such risks on behalf of my child.
3. Medical Authorization
In the event of an emergency, I authorize the Church and its representatives to secure emergency medical treatment for my child. I understand that I will be responsible for any expenses incurred for such treatment.
4. Release of Liability
I hereby release, waive, and hold harmless Grace Missionary Baptist Church Inc. of Gary, its pastors, employees, volunteers, agents, and affiliates from any and all claims, demands, actions, or causes of action, including but not limited to injury, illness, accident, or property loss, arising out of or related to my child's participation in this event, whether caused by negligence or otherwise.
